Discovery Call or First Inquiry

Once you’ve done some initial research and decided to reach out—either through a website inquiry or phone call—here’s what happens next, you’ll connect with a knowledgeable and compassionate Sales Director.

Our Sales Directors will:

  • Offer guidance and trusted resources to support your decision-making.
  • Help you navigate the next steps with confidence.
  • Provide emotional support, understanding the weight of this decision.
First Community Visit

When you visit a Northbridge community for the first time, you’ll meet key team members, including our Avita Memory Care Program Director.

The Sales Director and Memory Care Director will:

  • Partner with you from the start, offering clear communication and personal support.
  • Walk you through our Avita Memory Care neighborhood.
  • Show you what a typical day looks like for residents.
  • Share how our programs align with your loved one’s specific needs and preferences.
Assessment & Move-In

Once you’ve made the decision to move forward with making the move to a community, your loved one will receive a comprehensive assessment from our care team.

The assessment helps us:

  • Understand daily routines, likes/dislikes, and life history.
  • Recommend meaningful personal items to bring (e.g., a favorite blanket, cherished photos).
  • Pair residents with caregivers and peers who support comfort and connection.
  • This personalized information is used to create a customized care plan, shared and reviewed with your family.
Ongoing Support After Move-In

We provide continued support through:

  • Regular updates, photos, and positive stories about your loved one.
